About The Job
FT | Days or Nights| 3-2 split
Assesses patient’s needs, formulates a plan of care and provides care based on identified needs and physician orders according to standards, policy and procedure.
The RN develops and implements patient education plans. The RN communicates and collaborates with other team members to facilitate delivery of care.
Responsible for the patient for whom care is provided, self and peers as professional nurses, the charge nurse, clinical supervisor, unit director and CNO.
Works effectively toward collaborative relationships with medical staff and other members of the healthcare team and administrative personnel.
Maintains a cooperative working relationship with ancillary departments and the entire healthcare team.
Must be able to communicate verbally, in writing and utilization of the EMR.
Continuously provides expertise to the staff by assessing the quality and quantity of care and evaluating and intervening in the management of care problems, as necessary.
Oversees the unit's operations in the absence of the Director or CNO and collaborates with them on an as-needed basis.
Relies on experience and judgment to plan and accomplish goals

Requirements
MS RN license
Associates Degree in Nursing, Bachelors preferred
BLS certification, ACLS preferred
Step down / Progressive care experience preferred

The key responsibilities of a cardiac step down nurse include close monitoring and assessment of patients, administration of cardiac medications and treatments, patient education, and care coordination. Cardiac step down nurses continually monitor each patient's ECG, vital signs, and cardiovascular status.
However, the patient still requires closer supervision, more frequent vital sign checks, and a higher clinician-to-patient ratio than a regular ward can provide. This is where the step down ICU comes in with an intermediate level of care. The nurse patient ratios are lower than ICU but higher than the wards.
Being able to think critically and to make decisions will come a long way in this job. Patience. Being impatient and getting frustrated when new nurses come to you with questions or when other staff members come to check if they could switch shifts might be one of the biggest weaknesses for Charge Nurses.
Being a charge nurse is a big deal as it requires a great deal of knowledge and expertise in both patient care and leadership. It's considered an important step in nursing, with many facilities hiring or promoting team members specifically for this role.
To recap, the nursing hierarchy from bottom to top is: nursing aids, LPNs, Staff Nurses, Charge Nurses, Nursing Managers, Directors of Nursing and finally the Chief Nursing Officer. Nurses may go by many different titles depending on their specialty (or lack thereof).
Step-down, intermediate, PCU, and transitional care are other words used for progressive care. Patients in progressive units are often too sick for an acute care unit but might not meet criteria for critical care either. These patients are generally stable, but are at risk for deterioration.
PCUs and step-down units are both intermediate care settings between general units and ICUs. While the terms are sometimes used interchangeably, PCUs often care for moderately complex cases, while step down units assist patients transitioning from critical care to a lower level of care.
In terms of patient acuity and needs, the step-down unit is considered a step above med-surg. Patients in med-surg are stable, and although their conditions, injuries, or illnesses require hospitalization, they are not severe enough to necessitate intermediate care.
